• 2. Official transcript (non-Brown students only)
• 3. $300 non-refundable program deposit (checks made out to: Brown University)

The following forms and documents are required to complete your application: Deadline to submit documents: April 1, 2010

• 1. Summer Abroad Acceptance Form
• 2. General Travel Waiver
• 3. Immunization History Form (non-Brown students only)
• 4. Health Insurance Waiver Form (only for non-Brown students waiving the Brown student health insurance plan)
• 5. Disability and Medical Accommodations Form (as applicable)
• 6. Copy of your passport photo page (must be valid at least 3 months after the end of the program)
• 7. Copy of your travel itinerary (Do not purchase your flights until the program is confirmed to run on March 15)
